Why Monitor Pain Medication Therapy?

Extensive Customizable Panels

A comprehensive and actionable testing tool to identify patients at risk for therapy failure, ADE and to enhance patient drug adherence and medication record accuracy. Testing establishes an objective assessment of prescription and OTC (over the counter) usage.

Urine Testing

Urine sample used for LC/MS/MS confirmatory testing of over 100 drugs and their metabolites.

Oral Fluids

For patients who are unable to provide a urine sample, an oral fluid test can be done as an alternative. Oral fluid can be used instead of urine, donors can collect their own samples on-site and in the presence of a monitor, reducing the likelihood of tampering or a donor challenge later in the screening process.

d & l Isomer Testing

Many prescription and illicit drugs exist in two molecular forms, known as stereoisomers. While they appear to be the same molecule because they have the same chemical formula and very similar chemical properties, they are in fact different compounds and can have very different effects on the body.

Methamphetamine is an isomer drug and exists in two forms: d- and l-. The d- form is most frequently used as a prescription stimulant and appetite suppressant. The l- form is available over-the-counter and is the active ingredient in VICKS Vapor Rub. Illegally produced methamphetamine can contain mixtures of both with most being the d-Isomers. Both, d- and l- forms will register a positive methamphetamine result by both immunoassay and HPLC confirmations.

To differentiate between the two isomers, Capstone Healthcare performs an additional analysis, at no cost to the patient or provider, which reports the amount (or percentage) of d- isomer present in a specimen. A report that indicates more than 20% d-methamphetamine present indicates the use of illicit methamphetamine.

Designer Drugs

Urine test designed to detect a synthetic (chemically made) version of an illegal drug which has been altered slightly to avoid it being classified as illegal.

A biomarker screening that detects the presence of ethyl glucuronide, a breakdown product of ethanol, in urine samples. It can also detect the presence of ETG in your blood, hair and nails, although the urine test is the most widely used.

The purpose of an ETG test is to document required alcohol abstinence, but the urine test can only measure alcohol intake within the last one to three days.

Accurate Results

with quantification

Capstone Healthcare utilizes LC/MS/MS, the “Platinum Standard” for analytical quantitation. This technology filters analytes of interest from interfering substances which prevents false negatives and false positives. As a significant improvement over the existing MS methodology, LC/MS/MS provides rapid, unequivocal and quantitated tests of targeted drugs at trace levels in a complex matrix.
Every test sample undergoes three levels of review, this means you can trust Capstone Healthcare’ results when making pain management decisions for your patients.

Quality Control Measures

In addition to utilizing the most advanced technology for qualitative screening and quantitative confirmation, our quality control measures ensure that each result undergoes three layers of review. You can rely on a higher standard of precision for your results with Capstone Healthcare than with other leading laboratories.

Why Monitor Pain Medication Therapy?

Five key trends are occurring in the U.S. that propel effective pain medication management.

Delivery of chronic pain management is fragmented with traditional non-pain management specialties such as internal medicine and gynecology becoming a first line in pain management.

Chronic pain is a pandemic in the U.S. and is expected to increase due to the aging population.

The technological advancements of LC/MS/MS with nearly 100% sensitivity and specificity render pain medication monitoring a necessary and valuable tool in your pain management practice’s arsenal.

Studies report a high rate of non-adherence to pain medication regimens which is linked to poor pain control and higher overall healthcare costs.

The need for urine drug screens in chronic opioid therapy (COT) is recognized and recommended by the American Pain Society and American Academy of Pain Medicine.
